QUALIFICATION

Membership to this cooperative is open to anybody who:

a. Is at least 18 years of age but not more than 60 years of age.
b. Sagadian by blood, by birth or by marriage.
c. Must be mentally and physically fit to enter into a contract.
d. Pledges to undertake the responsibilities of a member.
e. Personally completes the required Pre-Membership Education Seminar (PMES)

The following are considered disqualifications from membership application: (Res. No. 123, s.2020)
a. Physically disabled without definite source of income
b. Blind (both eyes)
c. Deaf and/or mute who cannot read or write
d. Person with mental illness
e. Person who is undergoing dialysis and chemotherapy

 

MEMBERSHIP APPLICATION

a. Application for membership shall be made in writing in a form provided for the purpose.
b. The applicants shall submit the following requirements Birth Certificate, Barangay Certification of residency and employment;  TIN ID, and two (2) recent 1.5×1.5” I D pictures taken within the last 6 months from the submission of the application.

Additional requirements for the following applicant:
b.1. Employed

  • Certificate of Employment or;
  •  Any related documents (i.e. latest pay slip, service record)

b.2. Self- employed

  • Business Permit and/or;
  • Any related documents (i.e. Annual Income Tax Return)

c. The applicant must be guaranteed by at least two (2) members of good standing who personally know the applicant, one of whom is related within the 4th degree of affinity or consanguinity and the other a non-relative.
d. The duly accomplished form with the above stated requirements shall be submitted to the concerned Zonal Committee for evaluation, and submit to the New Accounts Clerk.
e. After the New Accounts Clerk conducted background investigation, applicants who passed will be endorsed to the Education Committee for PMES. Applicants found out to be past due with other cooperatives will be automatically disqualified while those who are delinquent within the last two years may be considered.
f. Attendance by proxy or walk-in is not allowed during PMES. Application forms shall be disposed off if the applicant has not complied with the PMES requirement within 3 months from date of submission thereof.
g. A PMES fee of Five Hundred Pesos (₱500.00) shall be collected from each applicant to cover PMES expenses.
h. A membership fee of Three Hundred Pesos (₱300.00) shall be collected upon the approval of membership application and deposit of the initial share capital/fixed deposit of not less than Two Thousand Five Hundred Pesos (P2,500.00) or 25 shares.
