1. Patients undergoing dialysis, chemotherapy and severely infected with Covid19
2. Patients needing major surgical treatment
𝗚𝗘𝗡𝗘𝗥𝗔𝗟 𝗚𝗨𝗜𝗗𝗘𝗟𝗜𝗡𝗘𝗦
1. Should be a member of SMPC
2. Patient undergoing dialysis can avail financial assistance not more than Three Thousand Pesos (3,000.00) or equivalent to one session. (Can be availed yearly)
3. A patient undergoing chemotherapy can avail financial assistance not more than 8,000.00 or equivalent to one session. (One time availment)
4. A patient severely infected with Covid19 can avail not more than 8,000.00 or equivalent to 1 vial (remdesivir). (One time availment). This applies to those confined in a hospital without compassionate special permit or not covered by Philhealth.
5. Patients needing major surgical procedure shall be given 5,000.00 or they are offered for a conduct of an activity for a cause especially for the person with the following conditions:
a. Family members SHOULD COMMIT themselves during the conduct of fund raising activity such as:
i. Help in the coordination with stakeholders or participating institution
ii. Help in marketing and advertisement
iii. Participate in the planning and implementation
b. The cooperative shall:
i. Spearhead the planning and design of the activity
ii. Provide assistance (technical, logistics, coordination, partnership and marketing)
iii. Recording and consolidation of donations/solicitations
iv. Bear the expense during the undertaking
v. Processing of permit
